Consultation and Applications to the communities are included.

The ten (10) steps involved in considering a transition to a retirement community setting such as the differences in styles of communities, questions to ask when touring communities, the transition process, considerations after move in, etc. are all outlined in this portfolio. Applications for residency, floor plans, activity schedules, and contact info for each community are included.

In addition, PSC will assist in filling out the applications, accompany the family on the tours of the communities, arrange for assessments to be made of the individual(s), assist the day of transition and provide one follow-up visit after a move occurs.
